EA 的declaration page有下载吗？

大家好，我找遍了EA的网站，好像没看到有官方关于declaration page的下载，就是要在CDR前面加上的这页，写的是：

Declaration:
The following declaration must be signed and presented
as part of your CDR submission:
‘All statements of fact in this report are true and correct and
I have made claims of acquired competencies in good faith.
The report is all my own work.................

大家都是怎么做的呢？直接从booklet上面copy下来，做成word档就可以了吗？
