MEDICARE OFFICE回复我是:
Dear Sir
Thank you for your email dated 14 March 2012 about enrolling with Medicare.
If your wife has applied for permanent residency in Australia, then she may be eligible for Medicare benefits.
To enrol in Medicare you must complete an Enrolment application and present it at a Medicare office or by mail. Applications by mail are only accepted in specific circumstances. The Enrolment application form is available on the Medicare website which can be accessed via the following link:
For your convenience I have attached a copy of the Enrolment Application
If you do not have a Medicare office within a reasonable distance from your place of residence, you may choose to send the form to us by mail. In this case, please photocopy your identification and have it certified by an approved person before mailing the Enrolment application form and supporting documents to the following address:
Department of Human Services
Medicare
Eligibility Department
GPO Box 9822
in your capital city
Note: Copies of the original documents must be certified as a true copy of the originals and will need to include the signature, address or office of one of the following:
A Justice of the Peace
Commissioner for Declarations
Commissioner of Affidavits
Person authorised under State Law
Solicitor
Registered Medical Practitioner
Bank Manager or Postal Manager
When lodging your application at a Medicare office please take original documents along with photocopies of the supporting documents.
The documents to supply are as follows;
Passport(all pages)
A letter from DIAC (Department of Immigration and Citizenship) confirming the date your application for permanent residency was accepted.
The receipt from DIAC for the lodgement of your application.
Supporting documents for your application- sponsorship information, permission to work documents or proof of relationship (ie spouse/children)
If permanent residency has already been granted Medicare must sight your passport showing your permanent residency visa or DIAC letter confirming permanent residency status has been granted and one other form of identification.
Children aged under 14 years are not required to attend the Medicare office. All persons 14 years and over are required to attend the Medicare office with their parents for their initial enrolment in the Medicare program.
