我会这么回答：

1）事前有一个好的Planning,争取把每一个关键点都列出来；
2）在做事的过程中小心，不马虎；
3）时候重复检查一遍，如果有错，分析原因，总结经验教训，如果没有，可以为下一提供一个范例。

补充一条，要在不同的部分加入check的环节，特别是critical point。

再就是要多拉人下水，不要自己一个人来扛。也就是让第三方做checking，一方面是segregation of duties，另一方面旁观者清，也容易发掘common sense的错误。

1.以退为进
所有的人都犯错，特别是新项目，新客户。
重要的是诚实面对错误，及时escalate问题，doc错误并改进流程，从错误中学习从而避免犯新错，犯更大的错。

2.占了便宜再卖乖
预防犯错的措施列出，这所有竞争者都会回答，出不了彩，但也得说。
尽管我们做了各种措施，但一定要随时提醒自己错误无法避免，所以要提前做好紧急预案，并做好Risk分析，提高个人的承受压力能力，在混乱的时候保持头脑冷静改正错误。

这是我下午自学一个课程时提高的"Classic" Risk Anallysis，给你参考下：
Identify relevant policies
- What existing risk tolerance policies are available?

Study the current environment
- Understand what strengths and weaknesses exist today

Perform requirements analysis
- Just how much availability is required?
- What is the acceptable likelihood of a long outage?

Hypothesize vulnerabilities
- What can possibly go wrong?

Identify and quantify risks
- The statistical probability of something going wrong over the life of the project 9 or the likely number of times something will go wrong over the life of the project0 multiplied by the cost of an occurrence

Evaluate countermeasures
- What will it take to reduce the risk (by reducing the likelihood or consequences of an occurrence) to an acceptable level

Make decisions, create a budget and a plan the environment

老大卫能犯的错误一定是高级错误，如何防范高级错误呢？

1.咱的工作性质是process orentation，大部分工作都有成熟/标准/安全的process可遵循，只要我按process去做就行了。

2.对于新的问题，我可以制定新的process，通过风险评估、风险管理的原理和规则去识别可能的风险点及对业务的危害、威胁，例举些ISO27001里有些风险评估、风险管理条款，这些条款虽然是信息安全方面的，但是源于英国的产业标准，应该是各行业通用的。

引用方法论：通过“计划-实施-检查-改进”"Plan-Do-Check-Act" (PDCA) model,去构架新的process
还有就是：
Due Care： involves carrying out the necessary steps to mitigate these risks，说的是，你应该去做的事情，像计划。
Due Diligence：is continual effort of making sure that the correct polices, procedures and standards are in place and being followed，说的是你要保证Due Care要做的那些事情一直在保持最新的状态，有点像审计，要保证Due Care在执行

answer the question in three fold:

Before the event
Good planning-list critical point
Risk analysis
Decide procedure
Communication

During the event
Keep tracking, double check in every step by using different method or different thinking style, ask third party to check
Communication

After the events
Learn the lessons,
Improve the procedure

以前上过一个项目管理课，有句话说得好，计划是个人就会制定，最主要的是如何很好地执行，所以做项目之前最最重要的事情就是识别风险，然后把要做的事情做在前面，充分降低以后的风险。

我在曾经有幸负责一个项目，实践了以上理念，非常受用：）
